Each skill updates its own row’s Last Run on completion. session-start-inject.sh reads this table and surfaces any row past Next Due at session start.

focus-ledger review is the one row that is not an /audit-* skill — a short human pass: read Core/Processes/Projects/Focus/focus-ledger.md and check it against the decision rule recorded there (>50% off after 10 rows → address why). Monthly is deliberate; the ledger needs ~10 rows before it says anything.
Re-ranking the Biggest Rocks and stamping rocks-refreshed: is NOT this row’s job — that is owned by Talbot’s weekly Plan: BIGGEST Rocks block (Fridays 1:30p, scheduled in Ecco + Google Calendar, 2026-08-20). One action, one owner: two cadences owning the same re-rank is the exact duplication this system exists to prevent.
Cadence values: fortnightly / monthly / quarterly — plain text, no cron syntax. Re-pick a skill’s cadence by editing its row directly; no code change needed.
Next Due = Last Run + cadence, computed by the skill itself when it writes Last Run (not auto-computed elsewhere — keeps this file a plain table, no hidden logic).
