Achilles Background

2. Response: The Rehab & ACTion Plan
Section titled “2. Response: The Rehab & ACTion Plan”Assistant: This is a classic application of your framework. You have the Knowledge (PT access), but the Behaviour (execution) is the point of failure. In chronic tendinopathy, the gap often exists because stretching is frequently the wrong solution—tendons need “loading” to become stiffer and stronger.

1. Awareness: The “Loading” Concept
Section titled “1. Awareness: The “Loading” Concept”- The Reality: Chronic Achilles issues are usually Tendinopathy (structural degeneration), not Tendonitis (acute inflammation).
- The Problem: Stretching a pathological tendon can actually cause “compressive loading” against the calcaneus (heel bone), which may aggravate the condition and explain your 10-year cycle of frustration.
- The Fix: Tendons remodel under tension. We use Heavy Slow Resistance (HSR) and Isometrics to “stiffen” the spring.
2. Understanding: The 3-Stage Protocol

Stage 1: Isometrics (Pain Relief & Initial Loading)
- The Move: Single-leg Calf Raise Hold. Stand on the ball of your right foot (at the top of a calf raise) and simply hold that position for 45–60 seconds.

- Why: This creates an “analgesic” effect, essentially numbing the pain while signaling the tendon to begin re-wiring itself without the stress of a full range of motion.
Stage 2: Isotonics (Structural Strength)
- The Move: Slow Heel Drops (Alfredson Protocol). 3 seconds to rise, 3 seconds to descend. Focus on the slow, controlled “eccentric” (lowering) phase.

- Why: This builds the structural integrity required to handle the explosive forces of badminton.
Stage 3: The Kinetic Chain (The Hips)
- The Move: The Couch Stretch. This is critical for you. It addresses the tight hip flexors that are pulling on your entire posterior chain, which contributes to your lower back issues.

- Adaptable: We start with Stage 1 (Isometrics) to kill pain. You only progress to the more difficult Stage 2 strength work if and when you value the additional performance for badminton.
- Simple: No equipment is needed. The “idea” is distilled down to: Stop stretching, start holding.
